Requirements

Candidates must be a licensed Physician, and possess a valid medical license in California. Experience in home healthcare or geriatric medicine is preferred, along with familiarity with diagnostic testing procedures and electronic medical record systems.

Responsibilities

The Per Diem Physician will conduct in-home health evaluations, engaging with patients and their families to assess health needs and bridge gaps in care. They will review medication and medical histories, perform physical evaluations, and order diagnostic tests as needed, while also providing education and support to patients and their care teams. The physician will also conduct virtual visits and collaborate with Signify Health and the patient’s health plan to ensure appropriate care coordination.
