Requirements

High School degree or equivalent plus a minimum of one year of experience in a Recovery-oriented role
Knowledge of the 12 steps for A.A
Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record
Professionalism and a commitment to excellence in care
Bilingual (Spanish proficient) a plus

Responsibilities

Work directly with mothers and children and assist with connection to community resources
Provide support in recognizing cravings, urges, triggers; and provide skill building including relapse prevention planning
Follow up with mothers up to six months post discharge, and provide links to supports as appropriate; and document
Prevent and assist with crisis situations; help reduce potential for relapse
Engagement and coordination with integrated team members for continuity of care, including written daily report when on shift
Record all documentation (UDS, med administration, individual sessions, group sessions, interventions, collateral phone contacts, and as needed releases of information, emergency contacts etc), in the EHR for continuity of resident records
Scan all written documentation into resident EHR (outside provider Urine Drug Screen, discharge paperwork, important documents, etc.)
Provide 24 hours, 7 days a week support and monitoring of mothers and children
