Requirements

  • Functions as part of the dialysis health care team under direct supervision of a licensed nurse
  • Complies with organization policies, procedures, training, and corporation, state, and federal regulations
  • Supports Quality Enhancement Program (QEP) and Continuous Quality Improvement (CQI) activities, including patient satisfaction and process improvements

Responsibilities

  • Assist in patient education on adequacy measures (OLC, AMP, UKM) and disease process/access
  • Welcome patients, inquire about wellbeing, evaluate vascular access for patency, cannulate access, and administer heparin as delegated or allowed by state law
  • Obtain pre- and post-treatment vital signs, weight, and vascular access evaluation
  • Initiate dialysis treatment per prescribed orders (QB, QD), evaluate intradialytic problems, intervene as prescribed, and monitor patient response
  • Obtain vital signs before reinfusion, perform discontinuation functions, document, evaluate patient prior to venous access termination (standing/sitting BP), obtain hemostasis, and apply dressings
  • Evaluate patient prior to discharge and record pre/post dialysis evaluations, weight, vital signs with initial identification
  • Calculate and enter patient-specific dialysis machine programming (UF goal, treatment time, UF modeling)
  • Ensure safety alarms are enabled (e.g., Narrow Venous Limits 160)
  • Record accurate/timely vital signs and treatment parameters on Hemodialysis Treatment Sheet with initial identification
  • Document patient treatment information (Hemodialysis Treatment Sheet, progress notes if applicable) and report significant changes to RN/supervisor
  • Observe patient, conduct machine safety checks, report changes/unusual findings to RN/supervisor
  • Document interventions for unusual patient status and patient response
  • Evaluate vascular access during treatment (arterial/venous pressures), intervene as needed, document/report unusual findings
  • Obtain prescribed laboratory testing and prepare specimens
  • Clean blood spills immediately with disinfectant per policy
  • Prepare, organize, and utilize supplies/equipment efficiently to prevent wastage
  • Monitor patients performing self-care under RN supervision
  • Assist in training direct patient care staff on appropriate programs
