Requirements

  • Functions under direct supervision of a licensed Home Therapies registered nurse (HT RN)
  • Compliance with organization policies, procedures, training, state laws, regulations, and federal agencies
  • Participation in Quality Enhancement Program (QEP), CQI activities, process improvement, and patient satisfaction initiatives

Responsibilities

  • Reinforces home hemodialysis (HHD) and/or peritoneal dialysis (PD) training as permitted by law
  • Identifies and welcomes patients, obtains vital signs, weights, and gathers information for Clinic Visit Intake form
  • Builds and maintains communication and collaboration with patients, care partners, HT RN, staff, providers, and team members
  • Obtains and reviews treatment records for completion, reports missing or abnormal data to HT RN
  • Assists with electronic home treatment/medication/oral nutritional supplement reconciliation, reports missing data to HT RN
  • Assists HT RNs with home visits and provides home support as applicable and permissible
  • Prepares supplies for laboratory testing, prepares specimens for shipping, tracks and ships transplant-related specimens, reinforces transplant education
  • Completes opening/closing daily chores and documents appropriately
  • Disinfects dialysis machines, chairs, tables, and equipment between and routinely
  • Prepares exam/training room for home dialysis training, treatment, or visits
  • Stocks exam, training, treatment, and storage rooms with supplies; manages supplies to maximize use and minimize waste
  • Schedules patient appointments, contacts patients/caregivers for reminders
  • Supports patient registration and use of connected health
  • Assists patients and care partners with supplies/inventory ordering and management
  • Maintains logs and tracking; may order and track clinic/office supplies
  • Participates in medical records, infection control, and audit processes
  • Collaborates during staff huddles or meetings
  • Collects documentation for patient admission, prepares and maintains patient charts
  • Performs clerical duties (answering phones, faxing, email, provider contacts)
  • Reinforces HHD training; sets up, tears down, programs, tests, and disinfects HHD machine
  • Troubleshoots HHD machinery/tools and connectivity issues
  • Prepares dialysate and dialysis supplies for HHD treatment per orders
  • Evaluates dialysis access before treatment, documents findings, reports abnormalities to HT RN
  • Initiates HHD treatment, monitors and documents patient status, vital signs, and machine data
  • Performs and documents safety checks during treatment per CMS and facility policy
  • Reports observed significant changes
