Requirements

  • Functions under direct supervision of a licensed nurse
  • Compliance with organization policies, procedures, training, and state/federal regulations
  • Knowledge of dialysis therapy, including Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), disease process/access, vascular access evaluation, cannulation, heparin administration (as delegated or allowed by state law)
  • Ability to setup and operate hemodialysis machines, including programming for Ultrafiltration (UF) goal, treatment time, UF modeling, and enabling safety alarms (e.g., Narrow Venous Limits 160)
  • Skills in obtaining vital signs, weights, vascular access evaluation, initiating/discontinuing dialysis treatments, monitoring intradialytic problems, hemostasis, and applying dressings
  • Ability to perform and document pre/post dialysis evaluations, machine safety checks, interventions for unusual patient status, arterial/venous monitoring, laboratory specimen preparation, and blood spill cleanup
  • Accurate and timely documentation on Hemodialysis Treatment Sheet and progress notes
  • Participation in Quality Enhancement Program (QEP), CQI activities, process improvement, and patient satisfaction initiatives
  • Maintenance of safe and clean working environment, efficient use of supplies/equipment

Responsibilities

  • Assist in patient education on adequacy measures (OLC, AMP, UKM) and disease process/access
  • Welcome patients, inquire about wellbeing, evaluate vascular access patency, cannulate access, administer heparin as allowed
  • Obtain pre/post treatment vital signs, weights, and vascular access evaluations
  • Initiate dialysis per orders (QB, QD), monitor patient response, evaluate intradialytic problems, intervene as prescribed
  • Obtain vital signs before reinfusion, discontinue treatment per procedures, evaluate patient (standing/sitting BP), achieve hemostasis, apply dressings
  • Evaluate patient prior to discharge, record pre/post evaluations, weights, vital signs
  • Calculate and enter patient-specific dialysis programming (UF goal, time, modeling), ensure safety alarms enabled
  • Record vital signs and treatment parameters on Hemodialysis Treatment Sheet
  • Document treatment information, complete sheets/progress notes, report significant changes to RN/supervisor
  • Observe patients, conduct machine safety checks, report unusual findings, document interventions and responses
  • Monitor vascular access during treatment (pressures), intervene as needed, report findings
  • Obtain prescribed lab tests, prepare specimens
  • Clean blood spills per policy
  • Prepare/organize supplies/equipment efficiently to prevent wastage
  • Monitor patients performing self-care under RN supervision
  • Assist in training direct patient care staff on programs
  • Support QEP/CQI activities and process improvements for quality goals
