Requirements

  • Functions under direct supervision of a licensed nurse in accordance with organization policies, procedures, training, and state/federal regulations
  • Compliance with corporate, state, and federal agency regulations
  • Knowledge of dialysis therapy, including adequacy measures (OLC, AMP, UKM), disease process/access, vascular access evaluation, cannulation, heparin administration (as delegated or allowed by state law)
  • Ability to setup and operate hemodialysis machines, including programming for UF goal, treatment time, UF modeling, and enabling safety alarms (e.g., Narrow Venous Limits 160)
  • Skills in patient monitoring, vital signs measurement (pre/post treatment, intradialytic), blood pressure evaluation (standing/sitting), hemostasis, and dressings
  • Accurate documentation skills (Hemodialysis Treatment Sheet, progress notes, vital signs, treatment parameters)
  • Ability to perform machine safety checks, intervene in intradialytic problems per physician orders, clean blood spills, handle lab specimens, and manage supplies/equipment efficiently
  • Participation in Quality Enhancement Program (QEP), CQI activities, process improvements, and patient satisfaction initiatives
  • Staff-related involvement, including recruitment process

Responsibilities

  • Assist in patient education on adequacy measures (OLC, AMP, UKM) and disease process/access
  • Welcome patients, inquire about wellbeing, evaluate vascular access for patency, cannulate access, administer heparin as allowed
  • Obtain pre/post treatment vital signs, weight, and vascular access evaluation
  • Initiate dialysis per prescribed orders (QB, QD), monitor patient response, evaluate intradialytic problems, intervene as prescribed
  • Obtain vital signs prior to reinfusion, discontinue treatment per procedures, evaluate patient (BP standing/sitting), achieve hemostasis, apply dressings
  • Evaluate patient prior to discharge, perform/record pre/post evaluations, weights, vital signs
  • Calculate and enter patient-specific dialysis programming (UF goal, time, modeling), ensure safety alarms enabled
  • Record vital signs/treatment parameters accurately/timely on Hemodialysis Treatment Sheet
  • Document patient treatment info, complete Treatment Sheet/progress notes, report significant changes to RN/supervisor
  • Observe patients, conduct machine safety checks, report changes/unusual findings to RN/supervisor
  • Document interventions for unusual patient status and patient response
  • Monitor vascular access (arterial/venous pressures), intervene as needed, document/report unusual findings
  • Obtain prescribed lab testing, prepare specimens
  • Clean blood spills immediately with disinfectant
  • Prepare/organize supplies/equipment efficiently to prevent wastage
  • Monitor patients performing self-care under RN supervision, assist as applicable
  • Participate in staff recruitment process
  • Maintain safe/clean working environment
  • Support QEP/CQI activities and process improvements for patient quality goals
