Requirements

Responsibilities

  • Drive FMS culture through values and customer service standards; provide outstanding customer service to external and internal customers
  • Develop and maintain effective relationships through communication; respond, resolve, and follow up on customer service issues
  • Assist in patient education on adequacy measures (OLC, AMP, UKM) and disease process/access
  • Welcome patients, inquire about well-being, report complaints/observations to nurse supervisor
  • Evaluate vascular access for patency pre-treatment, report unusual findings
  • Obtain pre- and post-treatment vital signs and weight, report unusual findings
  • Perform vascular access cannulation, report difficulties/unusual findings
  • Administer Heparin as delegated or allowed by state law
  • Initiate dialysis treatment per prescribed orders (blood flow QB, dialysate flow QD)
  • Evaluate intradialytic problems and intervene per physician order/direction
  • Monitor patient response to dialysis, report unusual findings
  • Obtain vital signs prior to reinfusion; perform functions for treatment discontinuation and document
  • Discontinue dialysis per procedures
  • Evaluate patient prior to venous access termination (standing/sitting BP)
  • Obtain hemostasis and apply dressings; evaluate patient prior to discharge, report findings
  • Perform and record pre- and post-dialysis evaluations, weight, vital signs with initial identification
  • Calculate and enter patient-specific dialysis machine programming (UF goal, treatment time, Na modeling, UF modeling)
  • Ensure safety alarms enabled (e.g., Narrow Venous Limits 160)
  • Record accurate/timely vital signs and treatment parameters on Hemodialysis Treatment Sheet with initial identification
  • Accurately document patient treatment information
  • Assist in maintaining safe and clean working environment
  • Setup and operate hemodialysis machines
  • Actively participate in process improvement activities for QEP and patient satisfaction
