Requirements

  • Ability to perform vascular access cannulation and administration of Heparin as delegated or allowed by state law
  • Knowledge of dialysis therapy, including Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), blood flow (QB), dialysate flow (QD), Ultrafiltration (UF) goal, treatment time, Sodium (Na) modeling, UF modeling, Narrow Venous Limits (NVL) 160, and machine safety alarms
  • Ability to calculate and enter individual patients' dialysis machine programming

Responsibilities

  • Assist in patient education regarding adequacy measures (OLC, AMP, UKM) and disease process/access
  • Welcome assigned patients and inquire about their well-being
  • Evaluate vascular access for patency pre-treatment and during treatment (including arterial/venous monitoring pressures)
  • Obtain necessary pre- and post-treatment vital signs, weight, and standing/sitting blood pressure
  • Perform vascular access cannulation
  • Administer Heparin as delegated or allowed by state law
  • Initiate, monitor, evaluate, and discontinue dialysis treatment according to prescribed orders and procedures, including reinfusion and hemostasis
  • Provide intervention for intradialytic problems as prescribed or directed
  • Calculate and enter dialysis machine programming for UF goal, treatment time, Na modeling, and UF modeling
  • Ensure appropriate safety alarms are enabled
  • Record accurate and timely information on vital signs, treatment parameters, Hemodialysis Treatment Sheet, and progress notes
  • Report significant information, changes in patient condition, or unusual findings to PCT III or nurse supervisor
  • Perform and document machine safety checks and interventions for unusual patient status
  • Obtain prescribed laboratory testing and prepare specimens
  • Maintain environmental integrity: clean and disinfect dialysis machine, surface, chair, equipment, and surrounding area between treatments
  • Perform all tasks for preparation of dialysis treatment, including pretreatment machine alarm testing (e.g., Pressure Hold)
  • Support Quality Enhancement Program (QEP), CQI activities, patient satisfaction, and process improvements
