Requirements

  • Functions under direct supervision of a licensed nurse
  • Compliance with organization policies, procedures, training, and state/federal regulations
  • Knowledge of dialysis therapy, including Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), disease process, and vascular access
  • Ability to perform vascular access evaluation, cannulation, and heparin administration as delegated or allowed by state law
  • Skills in setup and operation of hemodialysis machines, including programming for Ultrafiltration (UF) goal, treatment time, UF modeling, and enabling safety alarms (e.g., Narrow Venous Limits at 160)
  • Ability to monitor patients, evaluate responses, perform vital signs, weights, and hemostasis
  • Accurate documentation skills (e.g., Hemodialysis Treatment Sheet, progress notes)
  • Participation in Quality Enhancement Program (QEP), CQI activities, process improvements, and patient satisfaction initiatives
  • Maintenance of safe and clean working environment, including machine safety checks and blood spill cleanup

Responsibilities

  • Assist in patient education on adequacy measures, disease process, and access
  • Welcome patients, inquire about wellbeing, and evaluate vascular access pre-treatment
  • Obtain pre- and post-treatment vital signs, weights, and perform vascular access evaluations
  • Initiate, monitor, and discontinue dialysis treatments per prescribed orders (e.g., blood flow QB, dialysate flow QD)
  • Evaluate intradialytic problems, provide interventions as prescribed, and monitor patient responses
  • Calculate and enter patient-specific dialysis machine programming (UF goal, treatment time, UF modeling)
  • Record vital signs, treatment parameters, and evaluations accurately and timely with initial identification
  • Document all treatment-related information, including progress notes if applicable
  • Report significant patient changes, unusual findings, or conditions to registered nurse or supervisor
  • Perform and document interventions for unusual patient status
  • Monitor vascular access during treatment (arterial/venous pressures) and intervene/report as needed
  • Obtain prescribed laboratory testing and prepare specimens
  • Clean blood spills immediately with disinfectant
  • Prepare, organize, and utilize supplies/equipment efficiently to prevent wastage
  • Monitor patients performing self-care under RN supervision
  • Conduct machine safety checks and report issues
  • Assist in training direct patient care staff on applicable programs
  • Support QEP/CQI activities and process improvements for quality goals
