Requirements

  • Functions under direct supervision of a licensed nurse
  • Compliance with organization policies, procedures, training, and state/federal regulations
  • Knowledge of dialysis therapy, including Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), disease process/access
  • Ability to evaluate vascular access for patency, perform cannulation, and administer heparin as delegated or allowed by state law
  • Skills in setup and operation of hemodialysis machines, including programming for Ultrafiltration (UF) goal, treatment time, UF modeling, and enabling safety alarms (e.g., Narrow Venous Limits 160)
  • Ability to perform vital signs, weight measurements, vascular access evaluation, and hemostasis
  • Proficiency in documentation on Hemodialysis Treatment Sheet and progress notes
  • Capability to monitor patients, respond to intradialytic problems, and intervene as prescribed
  • Knowledge of machine safety checks and cleaning procedures (e.g., blood spills)
  • Ability to assist in training direct patient care staff

Responsibilities

  • Assist in patient education on adequacy measures and disease process/access
  • Welcome patients, inquire about wellbeing, and evaluate vascular access pre-treatment
  • Obtain pre- and post-treatment vital signs, weight, and perform vascular access evaluation
  • Initiate, monitor, and discontinue dialysis treatments per prescribed orders (e.g., QB, QD)
  • Evaluate and intervene in intradialytic problems as prescribed
  • Calculate and enter patient-specific dialysis machine programming (UF goal, treatment time, UF modeling)
  • Record vital signs, treatment parameters, and evaluations accurately and timely
  • Document all patient treatment information, including Hemodialysis Treatment Sheet and progress notes
  • Report significant changes or unusual findings to registered nurse or supervisor
  • Perform machine safety checks and observe patients
  • Evaluate vascular access during treatment, monitor pressures, and intervene/report as needed
  • Obtain prescribed laboratory testing and prepare specimens
  • Clean blood spills and maintain safe, clean environment
  • Prepare, organize, and utilize supplies/equipment efficiently
  • Monitor self-care patients under RN supervision
  • Assist in training direct patient care staff
  • Support Quality Enhancement Program (QEP), CQI activities, patient satisfaction, and process improvements
