Requirements

  • Functions under direct supervision of a licensed nurse
  • Compliance with organization policies, procedures, training, and corporation, state, and federal regulations
  • Knowledge of dialysis therapy, including setup and operation of hemodialysis machines
  • Ability to perform vascular access evaluation, cannulation, and heparin administration as delegated or allowed by state law
  • Skills in obtaining vital signs, weights, and performing pre/post dialysis evaluations
  • Proficiency in initiating, monitoring, and discontinuing dialysis treatments per prescribed orders (e.g., QB, QD, UF goal, treatment time, UF modeling)
  • Ability to enable safety alarms (e.g., Narrow Venous Limits 160)
  • Accurate documentation on Hemodialysis Treatment Sheet and progress notes
  • Observation and machine safety checks per facility policy
  • Preparation of laboratory specimens and cleaning of blood spills per policy
  • Efficient use of supplies and equipment to prevent wastage
  • Support for Quality Enhancement Program (QEP), CQI activities, patient satisfaction, and process improvements

Responsibilities

  • Assist in patient education on adequacy measures (OLC, AMP, UKM) and disease process/access
  • Welcome patients and inquire about wellbeing
  • Evaluate vascular access for patency, cannulate, and administer heparin as allowed
  • Obtain pre/post treatment vital signs, weights, and vascular access evaluations
  • Initiate dialysis per orders, monitor patient response, and intervene for intradialytic problems as prescribed
  • Obtain vital signs before reinfusion, discontinue treatment, evaluate patient (standing/sitting BP), achieve hemostasis, and apply dressings
  • Evaluate patient prior to discharge
  • Calculate and enter patient-specific dialysis programming (UF goal, time, modeling)
  • Record vital signs, treatment parameters, and ensure safety alarms are enabled
  • Document all treatment-related information and report significant changes to RN/supervisor
  • Conduct machine safety checks and document interventions for unusual patient status
  • Monitor vascular access during treatment (arterial/venous pressures) and intervene/report as needed
  • Obtain prescribed lab tests and prepare specimens
  • Clean blood spills immediately with disinfectant
  • Prepare/organize supplies and equipment efficiently
  • Monitor patients performing self-care under RN supervision
  • Assist in training direct patient care staff on applicable programs
  • Maintain safe and clean working environment
  • Actively participate in QEP and CQI activities to enhance patient outcomes
