Requirements

  • Functions under direct supervision of a licensed nurse
  • Compliance with organization policies, procedures, training, and state/federal regulations
  • Knowledge of dialysis therapy, including Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), disease process, and vascular access
  • Ability to perform vascular access evaluation, cannulation, and heparin administration as delegated or allowed by state law
  • Skills in operating hemodialysis machines, including setup, programming for Ultrafiltration (UF) goal, treatment time, UF modeling, and enabling safety alarms (e.g., Narrow Venous Limits 160)
  • Ability to obtain vital signs, weights, and perform pre/post dialysis evaluations
  • Accurate documentation skills for treatment sheets, progress notes, and patient information
  • Observation and intervention skills for patient and machine safety checks
  • Ability to collect laboratory specimens and handle blood spills with disinfectants
  • Efficient use of supplies and equipment to prevent wastage

Responsibilities

  • Assist in patient education on adequacy measures, disease process, and access
  • Welcome patients, inquire about wellbeing, and evaluate vascular access for patency
  • Obtain pre and post treatment vital signs, weight, and vascular access evaluation
  • Initiate, monitor, and discontinue dialysis treatments per prescribed orders, including blood flow (QB), dialysate flow (QD), and reinfusion
  • Evaluate intradialytic problems, provide interventions as prescribed, and monitor patient response
  • Calculate and enter dialysis machine programming for UF goal, treatment time, and UF modeling
  • Record vital signs, treatment parameters, and evaluations on Hemodialysis Treatment Sheet with initial identification
  • Document patient treatment information, including progress notes if applicable
  • Report significant changes or unusual findings to registered nurse or supervisor
  • Perform machine safety checks and document interventions for unusual patient status
  • Monitor arterial/venous pressures, provide interventions, and evaluate vascular access during treatment
  • Obtain prescribed laboratory testing and prepare specimens
  • Clean blood spills immediately with appropriate disinfectant
  • Prepare, organize, and utilize supplies/equipment efficiently
  • Monitor patients performing self-care under RN supervision
  • Assist in training direct patient care staff on appropriate programs
  • Support Quality Enhancement Program (QEP), CQI activities, patient satisfaction, and process improvements
  • Maintain safe and clean working environment and assist in its upkeep
