Requirements

  • Functions as part of the dialysis health care team under direct supervision of a licensed nurse
  • Compliance with organization policies, procedures, training, and corporation, state, and federal regulations
  • Knowledge of dialysis therapy, including setup and operation of hemodialysis machines
  • Ability to perform vascular access evaluation, cannulation, and heparin administration as delegated or allowed by state law
  • Skills in obtaining vital signs, weights, and performing pre/post dialysis evaluations
  • Capability to initiate, monitor, and discontinue dialysis treatments per prescribed orders
  • Proficiency in calculating and entering dialysis machine programming (e.g., UF goal, treatment time, UF modeling)
  • Ability to ensure safety alarms are enabled (e.g., Narrow Venous Limits 160)
  • Accurate and timely documentation on Hemodialysis Treatment Sheet and progress notes
  • Observation and machine safety checks per facility policy
  • Support for Quality Enhancement Program (QEP), CQI activities, patient satisfaction, and process improvements

Responsibilities

  • Assist in patient education on adequacy measures (OLC, AMP, UKM) and disease process/access
  • Welcome patients, inquire about wellbeing, and evaluate vascular access for patency
  • Obtain pre and post treatment vital signs, weights, and perform vascular access evaluations
  • Initiate dialysis treatment per orders (QB, QD), monitor patient response, and intervene for intradialytic problems
  • Discontinue treatment, obtain vital signs prior to reinfusion, evaluate hemostasis, and apply dressings
  • Perform and record pre/post dialysis evaluations, weights, vital signs with initial identification
  • Calculate and enter patient-specific dialysis programming for UF goal, treatment time, and modeling
  • Record vital signs and treatment parameters accurately on Hemodialysis Treatment Sheet
  • Document patient treatment information and report significant changes to RN or supervisor
  • Conduct machine safety checks, monitor vascular access (arterial/venous pressures), and intervene/report unusual findings
  • Obtain prescribed laboratory testing and prepare specimens
  • Clean blood spills immediately with disinfectant
  • Prepare, organize, and utilize supplies/equipment efficiently to prevent wastage
  • Monitor patients performing self-care under RN supervision
  • Assist in training direct patient care staff
  • Maintain safe and clean working environment
  • Actively participate in QEP and CQI activities to enhance patient outcomes
