Requirements

  • Supports Quality Enhancement Program (QEP) and CQI activities
  • Actively participates in process improvement activities
  • Knowledge of Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), disease process, and vascular access
  • Ability to perform vascular access evaluation, cannulation, heparin administration (as delegated or allowed by state law), vital signs, weights, and dialysis machine setup/operation/programming
  • Ability to monitor patients, evaluate responses, intervene as prescribed, and document accurately

Responsibilities

  • Drive FMS culture through values and customer service standards; provide outstanding customer service to all customers
  • Develop and maintain effective relationships through communication; respond, resolve, and follow up on customer service issues
  • Assist in patient education on adequacy measures, disease process, and access
  • Welcome patients, inquire about well-being, report complaints/observations to nurse supervisor
  • Evaluate vascular access for patency pre-treatment, report unusual findings
  • Obtain pre- and post-treatment vital signs and weights; report unusual findings
  • Perform vascular access cannulation; report difficulties/unusual findings
  • Administer Heparin as delegated/allowed
  • Initiate dialysis treatment per prescribed orders (QB, QD, UF goal, treatment time, Na modeling, UF modeling)
  • Enable safety alarms (e.g., Narrow Venous Limits 160)
  • Monitor intradialytic problems, provide interventions as prescribed/directed
  • Monitor patient response to therapy; report unusual findings
  • Obtain vital signs prior to reinfusion; perform discontinuation functions and document
  • Discontinue treatment per procedures; evaluate patient (standing/sitting BP) prior to termination of venous access
  • Obtain hemostasis and apply dressings
  • Evaluate patient prior to discharge; report unusual findings
  • Perform and record pre- and post-dialysis evaluations, weights, vital signs with initial identification
  • Calculate and enter patient-specific dialysis machine programming
  • Record accurate/timely vital signs and parameters on Hemodialysis Treatment Sheet with initial identification
  • Document patient treatment information (Hemodialysis Treatment Sheet, progress notes if applicable)
  • Report significant information/changes in patient condition to nurse supervisor
  • Observe patient and conduct machine safety checks
  • Assist in maintaining safe and clean working environment
