Requirements

  • Work under direct supervision of a licensed nurse
  • Comply with organization policies, procedures, training, and corporate, state, and federal regulations
  • Knowledge of adequacy measures including Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), disease process, and vascular access
  • Eligibility (delegated or allowed by state law) to perform vascular access cannulation and heparin administration
  • Ability to calculate and program dialysis machines for Ultrafiltration (UF) goal, treatment time, and UF modeling
  • Knowledge of safety alarms including Narrow Venous Limits (NVL) 160
  • Knowledge of Quality Enhancement Program (QEP) and Continuous Quality Improvement (CQI) activities

Responsibilities

  • Set up and operate hemodialysis machines
  • Assist in maintaining a safe and clean working environment
  • Support organizational commitment to QEP and CQI activities, including patient satisfaction, and participate in process improvement
  • Assist in providing patient-specific education on adequacy measures, disease process, and access
  • Welcome assigned patients and inquire about their wellbeing since last treatment
  • Evaluate vascular access for patency pre-treatment
  • Obtain pre- and post-treatment vital signs, weight, and vascular access evaluation
  • Initiate dialysis treatment per prescribed orders, including blood flow (QB) and dialysate flow (QD)
  • Evaluate intradialytic problems and intervene as prescribed or directed
  • Monitor patients' response to dialysis therapy
  • Obtain vital signs prior to reinfusion and perform functions for treatment discontinuation
  • Discontinue dialysis per procedures, evaluate patient (standing and sitting blood pressure) prior to venous access termination
  • Obtain hemostasis and apply appropriate dressings
  • Evaluate patient prior to discharge
  • Record pre- and post-dialysis evaluations, weight, and vital signs with initial identification
  • Enter patient-specific dialysis programming for UF goal, treatment time, and UF modeling
  • Ensure appropriate safety alarms are enabled
  • Record accurate and timely vital signs and treatment parameters on Hemodialysis Treatment Sheet with initial identification
  • Complete accurate documentation of treatment information, including Hemodialysis Treatment Sheet and progress notes if applicable
  • Report significant information or changes in patient condition to registered nurse or supervisor
  • Observe patients and conduct machine safety checks per facility policy; report changes or unusual findings
  • Perform and document interventions for unusual patient status and patient response
  • Evaluate vascular access during treatment (arterial and venous pressures), intervene as needed, document, and report unusual findings
  • Obtain prescribed laboratory testing and prepare specimens for collection
  • Clean all blood spills immediately with appropriate disinfectant per facility policy
  • Prepare, organize, and utilize supplies and equipment efficiently to prevent wastage
  • Monitor patients performing self-care under RN supervision
  • Assist in training applicable direct patient care staff on appropriate procedures
