Requirements

  • Functions under direct supervision of a licensed nurse
  • Compliance with organization policies, procedures, training, and state/federal regulations
  • Knowledge of dialysis therapy, including Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), disease process/access
  • Ability to perform vascular access evaluation, cannulation, and heparin administration as delegated or allowed by state law
  • Skills in setup and operation of hemodialysis machines
  • Ability to calculate and enter dialysis machine programming for Ultrafiltration (UF) goal, treatment time, and UF modeling
  • Knowledge of safety alarms, including Narrow Venous Limits (NVL) 160
  • Accurate documentation skills for vital signs, treatment parameters, Hemodialysis Treatment Sheet, and progress notes
  • Ability to conduct machine safety checks and patient monitoring

Responsibilities

  • Assist in patient education on adequacy measures (OLC, AMP, UKM) and disease process/access
  • Welcome patients, inquire about wellbeing, evaluate vascular access for patency, cannulate access, administer heparin
  • Obtain pre- and post-treatment vital signs, weight, and vascular access evaluation
  • Initiate dialysis treatment per prescribed orders (blood flow QB, dialysate flow QD)
  • Monitor patients’ response to dialysis, evaluate intradialytic problems, provide interventions as prescribed
  • Discontinue dialysis treatment, obtain vital signs prior to reinfusion, evaluate patient prior to termination of venous access (standing/sitting BP), achieve hemostasis, apply dressings
  • Evaluate patient prior to discharge, perform/record pre- and post-dialysis evaluations
  • Calculate and enter patient-specific dialysis programming (UF goal, treatment time, UF modeling), ensure safety alarms enabled
  • Record vital signs and treatment parameters on Hemodialysis Treatment Sheet
  • Document patient treatment information, report significant changes to RN/supervisor
  • Observe patient, conduct machine safety checks, report unusual findings
  • Perform/document interventions for unusual patient status
  • Monitor vascular access during treatment (arterial/venous pressures), intervene as needed, document/report findings
  • Obtain prescribed laboratory testing, prepare specimens
  • Clean blood spills immediately with disinfectant
  • Prepare/organize supplies and equipment to prevent wastage
  • Monitor patients performing self-care under RN supervision
  • Assist in training direct patient care staff
  • Maintain safe/clean working environment
  • Support Quality Enhancement Program (QEP), CQI activities, patient satisfaction, and process improvements
