Requirements

  • Functions under direct supervision of a licensed nurse in accordance with FMCNA policies, procedures, training, and compliance with corporation, state, and federal regulations
  • Supports FMCNA Quality Enhancement Program (QEP) and CQI activities, including patient satisfaction and process improvement for FMC Quality Enhancement Goals

Responsibilities

  • Assist in patient education on adequacy measures (OLC, AMP, UKM) and disease process/access
  • Welcome patients, inquire about well-being, evaluate vascular access for patency
  • Obtain pre- and post-treatment vital signs and weight
  • Perform vascular access cannulation and administration of Heparin as delegated or allowed by state law
  • Initiate dialysis treatment per prescribed orders (QB, QD), evaluate intradialytic problems, provide interventions as prescribed
  • Monitor patients' response to dialysis, obtain vital signs prior to reinfusion, discontinue treatment per procedures
  • Evaluate patient prior to termination of venous access (standing/sitting BP), obtain hemostasis, apply dressings
  • Evaluate patient prior to discharge, perform/record pre- and post-dialysis evaluation, weight, vital signs with initial identification
  • Calculate and enter patient-specific dialysis machine programming (UF goal, treatment time, Na modeling, UF modeling)
  • Ensure safety alarms enabled (e.g., Narrow Venous Limits 160)
  • Record accurate/timely vital signs and treatment parameters on Hemodialysis Treatment Sheet with initial identification
  • Document patient treatment information, complete Hemodialysis Treatment Sheet and progress notes if applicable
  • Report significant information/change in patient condition to PCT III or nurse supervisor
  • Observe patient, conduct machine safety checks, report changes/unusual findings
  • Perform/document interventions for unusual patient status and patient response
  • Evaluate vascular access during treatment (arterial/venous pressures), provide interventions, document/report unusual findings
  • Obtain prescribed laboratory testing, prepare specimens for collection
  • Maintain environmental integrity/aesthetics, ensure areas safe and clean
  • Clean/disinfect dialysis machine, surface, chair, equipment, surrounding area between treatments
  • Perform tasks for preparation of dialysis treatment, document as appropriate
  • Conduct pretreatment dialysis machine alarm testing (e.g., Pressure Hold)
