Requirements

  • Registered professional nurse (RN) with entry-level designation into the Clinical Advancement Program (CAP RN 1)
  • Ability to perform all essential functions under the direction of the Clinical Manager and with guidance from Educator, Preceptor, or higher CAP RN levels for the first 6 months
  • Expected to complete requirements to advance to the next CAP level within 24 months from date of hire
  • Compliance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations
  • Ability to meet physical demands and working conditions of the role (e.g., encounters while performing essential functions; reasonable accommodations for disabilities)

Responsibilities

  • Perform ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment, document in medical record, adjust/modify treatment plan, and notify Team Leader, Charge Nurse, Clinical Manager, or Physician as needed
  • Recognize aspects and implications of patient status that vary from normal and report/collaborate with appropriate health team members
  • Assess daily assigned patient care needs and collaborate with direct and ancillary patient care staff
  • Direct and provide, in collaboration with staff, all aspects of safe and effective chronic hemodialysis therapy delivery to assigned patients
  • Administer medications as prescribed or per approved algorithm(s), document justification and effectiveness
  • Ensure correct laboratory collection, processing, shipping; reschedule missed/insufficient collections
  • Ensure physician orders are entered and acknowledged in the Medical Record
  • Initiate or assist with emergency response measures
  • Delegate tasks to direct patient care staff (e.g., LVN/LPNs, Patient Care Technicians) and monitor for appropriate techniques and adherence to policy/procedures
  • Promote infection control, equipment, and environmental safety
  • Assess, collaborate, and document patient/family basic learning needs; provide initial and ongoing education
  • Identify expected outcomes, document/update nursing assessment and plan of care via collaboration with Interdisciplinary Team
  • Ensure patient awareness of transplant and treatment modality options
  • Make referrals to Social Worker and Registered Dietitian for patient education/counseling
  • Check availability of adequate emergency equipment and supplies
  • Assist with medication tracking and inventory
  • Serve as a resource for health care team, participate in staff training and orientation of new staff
  • Participate in all required staff meetings
  • Support and promote FMCNA's mission, vision, values, and customer service philosophy
  • Perform all other duties as assigned by supervisor
