Requirements

  • RN CAP 2 designation (entry level for new employees meeting criteria or advancement from RN CAP 1)
  • Accountability for provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
  • Ability to delegate appropriate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
  • Participation in decision-making, teaching, leadership functions, and quality improvement activities
  • Completion of CAP requirements to maintain or advance
  • Physical ability to perform direct patient care involving heavy lifting, moving patients, assisting with ambulation, frequent prolonged standing, bending, moving machines/equipment up to 200 lbs. (with assistance), lifting chemical/water solutions up to 30 lbs. as high as 5 feet
  • Work in a health care facility environment with air temperature control and moderate noise levels (may be exposed to related conditions)

Responsibilities

  • Perform all duties in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state, and federal regulations
  • Perform essential functions under direction of Supervisor and with guidance from Clinical Educator, Preceptor, or in collaboration with other RNs
  • Conduct ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment, document in medical record, adjust/modify treatment plan as indicated, and notify Team Leader, Charge Nurse, Supervisor, or Physician as needed
  • Assess, collaborate, and document patient/family basic learning needs; provide initial and ongoing education to patients and family
  • Direct and provide, in collaboration with direct and ancillary staff, all aspects of daily safe and effective hemodialysis therapy delivery to assigned patients
  • Administer medications as prescribed or per approved algorithm(s), document medical justification and effectiveness
  • Initiate or assist with emergency response measures
  • Serve as resource, leader, coach, mentor, and role model for new and incumbent employees by example in behavior, work habits, and attitudes
  • Ensure correct laboratory collection, processing, shipping procedures; reschedule missed or insufficient collections
  • Identify expected outcomes, document, and update nursing assessment and plan of care through collaboration with Interdisciplinary Team
  • Ensure patient awareness of transplant and treatment modality options
  • Participate in education and quality improvement projects at facility and area level as directed by Supervisor
  • Serve as Preceptor to new employees
  • Perform all other duties as assigned by Supervisor
