Responsibilities

  • Provides and coordinates clinically competent care for an assigned group of patients within the Chronic Kidney Disease (CKD) healthcare team
  • Participates in decision-making, teaching, leadership, and quality improvement activities
  • Performs assessment, planning, intervention, and evaluation of patient care
  • Collects and analyzes patient data pre, during, and post hemodialysis treatment
  • Documents patient data in the medical record
  • Makes adjustments to the treatment plan as needed
  • Notifies Team Leader, Charge Nurse, Clinical Manager, or Physician as needed
  • Recognizes and reports patient status variations
  • Collaborates with healthcare team members for input
  • Provides daily hemodialysis therapy to assigned patients
  • Administers medications as prescribed or in accordance with approved algorithms, documenting justification and effectiveness
  • Ensures correct laboratory collection, processing, and shipping procedures
  • Reschedules missed or insufficient laboratory collections
  • Enters physician orders and acknowledges them in the Medical Record
  • Initiates or assists with emergency response measures
  • Delegates appropriate tasks to direct patient care staff (LVN/LPNs and Patient Care Technicians)
  • Monitors patient care staff for appropriate techniques and adherence to policy
  • Assesses patient/family learning needs
  • Provides initial and ongoing education to patients and family
  • Promotes infection control, equipment, and environmental safety
