Requirements

  • Registered Nurse (RN) license in good standing
  • Completion of the Clinical Advancement Program (CAP) requirements
  • Knowledge of hemodialysis procedures and patient care principles
  • Ability to assess, plan, intervene, and evaluate patient care
  • Ability to collaborate with and direct other patient care staff (RNs, LVNs/LPNs, Patient Care Technicians)

Responsibilities

  • Performs ongoing, systematic collection and analysis of patient data pre-, during-, and post-hemodialysis treatment
  • Documents patient information accurately in the medical record
  • Makes adjustments to the treatment plan as needed and notifies appropriate personnel
  • Performs assessments and documents patient/family’s basic learning needs
  • Provides initial and ongoing education to patients and families
  • Directs and provides aspects of daily hemodialysis therapy delivery
  • Administers medications as prescribed or in accordance with approved algorithms, documenting justification and effectiveness
  • Initiates or assists with emergency response measures
  • Serves as a resource for the healthcare team
  • Participates in staff training and orientation of new staff
  • Collaborates with the Interdisciplinary Team to update nursing assessments and care plans
  • Ensures patient awareness related to transplant and treatment modality options
  • Ensures correct laboratory collection, processing, and shipping procedures are performed
  • Reschedules missed or insufficient laboratory collections
  • Performs all other duties as assigned by the Supervisor
