Entry level designation into the Clinical Advancement Program (CAP) - required to complete CAP requirements to advance
Accountable and responsible for provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
Ability to delegate appropriate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
Participation in decision-making, teaching, leadership functions, and quality improvement activities
Perform all duties in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations
Ability to perform physical demands: heavy lifting, moving patients, assisting with ambulation, frequent prolonged standing, bending, moving machines/equipment up to 200 lbs. (with assistance), lifting chemical/water solutions up to 30 lbs. as high as 5 feet
Tolerance for work environment: health care facility with air temperature control, moderate noise, exposure to infectious/contagious diseases, desk/computer work, patient/staff/physician interaction, possible travel

Responsibilities

Performs all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor, or in collaboration with other Registered Nurses
Performs ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment for assigned patients, documents in medical record, adjusts treatment plan as indicated, notifies Team Leader/Charge Nurse/Supervisor/Physician as needed
Assesses, collaborates, and documents patient/family’s basic learning needs to provide initial and ongoing education
Directs and provides, in collaboration with direct and ancillary staff, all aspects of daily safe and effective hemodialysis therapy delivery to assigned patients
Administers medications as prescribed or per approved algorithm(s), documents justification and effectiveness
Initiates or assists with emergency response measures
Serves as a resource for health care team, participates in staff training and orientation of new staff
Ensures correct laboratory collection, processing, shipping; reschedules missed/insufficient collections
Identifies expected outcomes, documents and updates nursing assessment and plan of care through collaboration with Interdisciplinary Team
Ensures patient awareness of transplant and treatment modality options
Performs all other duties as assigned by Supervisor
