Requirements

  • Entry level designation into the Clinical Advancement Program (CAP) - required to complete CAP requirements to advance
  • Accountable and responsible for provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
  • Ability to delegate appropriate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
  • Participation in decision-making, teaching, leadership functions, and quality improvement activities
  • Perform all duties in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations
  • Ability to perform physical demands: heavy lifting, moving patients, assisting with ambulation, frequent prolonged standing, bending, moving machines/equipment up to 200 lbs. (with assistance), lifting chemical/water solutions up to 30 lbs. as high as 5 feet
  • Tolerance for work environment: health care facility with air temperature control, moderate noise, exposure to infectious/contagious diseases, desk/computer work, patient/staff/physician interaction, possible travel

Responsibilities

  • Performs all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor, or in collaboration with other Registered Nurses
  • Performs ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment for assigned patients, documents in medical record, adjusts treatment plan as indicated, notifies Team Leader/Charge Nurse/Supervisor/Physician as needed
  • Assesses, collaborates, and documents patient/family’s basic learning needs to provide initial and ongoing education
  • Directs and provides, in collaboration with direct and ancillary staff, all aspects of daily safe and effective hemodialysis therapy delivery to assigned patients
  • Administers medications as prescribed or per approved algorithm(s), documents justification and effectiveness
  • Initiates or assists with emergency response measures
  • Serves as a resource for health care team, participates in staff training and orientation of new staff
  • Ensures correct laboratory collection, processing, shipping; reschedules missed/insufficient collections
  • Identifies expected outcomes, documents and updates nursing assessment and plan of care through collaboration with Interdisciplinary Team
  • Ensures patient awareness of transplant and treatment modality options
  • Performs all other duties as assigned by Supervisor
