Requirements

  • Registered Nurse (RN) license in good standing
  • Basic Life Support (BLS) certification
  • Clinical Advancement Program (CAP) requirements to be completed
  • Ability to perform systematic data collection and analysis related to hemodialysis treatment
  • Knowledge of nursing assessment and plan of care development

Responsibilities

  • Performs ongoing systematic collection and analysis of patient data pre-, during-, and post-hemodialysis treatment
  • Documents adjustments to the treatment plan and notifies Team Leader, Charge Nurse, Supervisor, or Physician as needed
  • Assesses patient/family’s basic learning needs and provides initial and ongoing education
  • Collaborates with direct and ancillary patient care staff to deliver safe and effective hemodialysis therapy
  • Administers medications as prescribed or in accordance with approved algorithm(s), documenting medical justification and effectiveness
  • Initiates or assists with emergency response measures
  • Serves as a resource for the healthcare team, participates in staff training, and assists with orientation of new staff
  • Ensures correct laboratory collection, processing, and shipping procedures are performed and reschedules missed collections
  • Identifies expected outcomes, documents, and updates the nursing assessment and plan of care through collaboration with the Interdisciplinary Team
  • Ensures patient awareness related to transplant and treatment modality options
  • Performs all other duties as assigned by the Supervisor
