Candidates must be an APRN with at least 3 years of experience in outpatient clinical practice. An active and clear RN and APRN license is required in one of the following states: Pennsylvania, Georgia, Mississippi, Missouri, Illinois, Hawaii, Kentucky, Tennessee, North Carolina, South Carolina, Ohio, Indiana, Arkansas, Washington, or Arizona. A valid BLS certification is mandatory at the time of hire and must be maintained throughout employment. Experience in primary care, family medicine, geriatrics, or palliative care is desired, as is experience with Medicare annual wellness visits, in-home comprehensive visits with elderly or Medicare patients, and telehealth video platforms. Understanding of managed care, including STARS/HEDIS measures, coding clinical comorbidities, and identifying care gaps, is a plus.
The Nurse Practitioner will conduct primarily telehealth video visits with members, with potential for a limited number of home-based visits. Responsibilities include performing comprehensive assessment visits (CAVs) to review diagnoses, medications, and assess for quality of care interventions, social needs, and home health/DME requirements. The role involves close collaboration with the member's care team, including PCPs, specialists, and other Devoted team members such as pharmacy, clinical nursing, and social work, as well as family members and caregivers, to coordinate care and develop collaborative care plans.
