Requirements

BA/BS in a health-related field and minimum of 5 years of clinical experience; or any combination of education and experience which would provide an equivalent background
Current, unrestricted RN license in applicable state(s)
Multi-state licensure required if providing services in multiple states
Willingness to complete an on-line pre-employment skills assessment within 48 hours
Location within reasonable commuting distance from Jonesboro, GA area

Responsibilities

Assess, develop, implement, coordinate, monitor, and evaluate care plans for members with complex and chronic care needs to optimize health care across the care continuum
Ensure member access to services appropriate to their health needs
Conduct assessments to identify individual needs and develop specific care management plans
Implement care plans by facilitating authorizations/referrals within benefits structure or extra-contractual arrangements
Coordinate internal and external resources to meet identified needs
Monitor and evaluate effectiveness of care management plans and modify as necessary
Assist in problem solving with providers, claims, or service issues
Coordinate referrals to local and statewide resources including behavioral health, housing, transportation, and food assistance
Partner with community health organizations, advocacy groups, and outreach teams to strengthen member connections
Plan, coordinate, and deliver educational events in collaboration with community partners, employers, or local health organizations
Provide group-based and one-on-one education on chronic conditions, medication adherence, preventive screenings, nutrition, and self-care
Support initiatives that address health equity and promote culturally responsive care
Assist with development of utilization/care management policies and procedures
Chair and schedule meetings, present cases for discussion at Grand Rounds/Care Conferences, and participate in interdepartmental and/or cross brand workgroups
Develop focused skill set on specific disease processes or traumatic injuries and function as preceptor for new care management staff
Participate in department audit activities
Perform duties primarily in the field with some telephonically, including travel to health plan-sponsored events, individual and group presentations (no in-home or facility-based visits)
