Responsibilities

Provide individual and group therapy to a panel of complex SMI patients
Provide continuous, regular support as patients navigate recovery, both in person and occasionally virtually
Deliver evidence-based care in a holistic, wrap-around care model
Collaborate with patient care teams and behavioral health providers to ensure integrated care
Support a panel of complex members through individual and group therapy sessions
Provide psychoeducation and support to patients' families
Partner with clinical staff, peer specialists, primary care providers, and community partners
Help members navigate the health and social services systems, connecting patients to resources and the community
Initiate, establish, and maintain relationships with patients and families rooted in trust and rapport
