Requirements

  • Master's degree in Social Work (MSW) - entry level role
  • Current state license as a Master Social Worker (MSW) permitting clinical practice within scope
  • Knowledge of social work theory of human behavior and accepted methods of social work practice
  • Knowledge of Chronic Kidney Disease's emotional, psychological, and behavioral impact on patients and families
  • Familiarity with FKC patient education programs, quality of life measurement instruments, and FKC Quality Indicators/Outcomes/QAI activities
  • Current knowledge of local vocational/educational rehabilitation programs and community resources (e.g., home health services)
  • Adherence to FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy

Responsibilities

  • Assess patients' psychosocial status, strengths, and areas of need as part of comprehensive patient assessment in collaboration with the interdisciplinary team
  • Participate in care planning with patients and healthcare team to identify interventions for rehabilitation, treatment goals, and improved quality of life
  • Utilize FKC patient education programs, social work theory/methods, and interventions to address barriers and meet treatment goals
  • Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status/quality of life
  • Provide supportive counseling services to patients within scope of clinical training and state license
  • Offer educational and goal-directed counseling to transplant-seeking patients
  • Provide information and assist with referrals to community resources (e.g., home health, vocational rehabilitation) for optimal treatment outcomes
  • Assist patients with access to vocational rehabilitation to maintain/attain employment or education
  • Assess patient awareness of advance directives, facilitate access/discussion of forms, and discuss wishes with team/family
  • Participate in discussions of patient DNR status with physician, nurse, patient, and family to ensure informed decisions
  • Document MSW interactions and interventions provided to patient/family
  • Provide psychosocial support/Social Work Focused Interventions for non-adherence, quality outcomes, and quality of life concerns based on acuity
  • Participate in monthly Quality review meetings, reporting on indicators like Missed/Shortened Treatments, Quality of Life Trends, and Service Recovery
  • Assess patient knowledge of kidney disease for barriers to adherence and provide tailored education with patient, family, and team
