Requirements

  • Master's degree in Social Work (MSW) - entry level role
  • Current state license as a Master Social Worker (as permitted within scope of clinical training and state license)
  • Knowledge of Social Work Theory of Human Behavior and accepted methods of social work practice
  • Familiarity with FKC patient education programs, social work focused interventions, and quality of life measurement instruments
  • Current knowledge of local vocational/educational rehabilitation programs
  • Knowledgeable of FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy
  • Adherence to documentation requirements based on MSW interactions and interventions

Responsibilities

  • Assess patients' psychosocial status, strengths, and areas of need as part of the comprehensive patient assessment in collaboration with the interdisciplinary team
  • Participate in care planning with the patient and healthcare team to identify effective interventions for rehabilitation, treatment goals, and improved quality of life
  • Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life
  • Provide supportive counseling services to patients within scope of clinical training and state license
  • Provide educational and goal-directed counseling to patients seeking transplant
  • Provide information and assist with referrals to community resources (e.g., home health services, vocational rehabilitation) to facilitate optimal treatment outcomes
  • Assist patients with referral and access to vocational rehabilitation to enable employment, re-employment, or education
  • Assess patient awareness of advance directives, assist with accessing forms/information, and facilitate discussions with the healthcare team and family/support persons
  • Participate in discussions of patient DNR status with physician, nurse, patient, and family to ensure informed decisions
  • Provide psychosocial support and/or Social Work Focused Interventions to address non-adherence, quality outcomes, and quality of life concerns based on acuity level
  • Participate in monthly Quality review meetings, reporting on quality indicators (e.g., Missed/Shortened Treatments, Quality of Life Trends, Service Recovery)
  • Assess patient knowledge of kidney disease for barriers affecting adherence and provide tailored education with patient, family, and healthcare team
  • Document based on MSW interactions and interventions provided to patient and/or family
  • Inform, educate, and support staff on the emotional, psychological, and behavioral impact of Chronic Kidney Disease on patients and families
  • Actively participate in process improvement activities and QAI activities related to patient satisfaction, quality of life, and FKC Quality Goals
