Requirements

Responsibilities

  • Assess patients' psychosocial status, strengths, and areas of need as part of comprehensive patient assessment
  • Collaborate with patient and health care team to identify effective interventions for rehabilitation, treatment goals, and improved quality of life
  • Utilize patient education programs, social work theory/methods, interventions, and quality of life instruments in assessment and care planning to address barriers
  • Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life
  • Assess patient knowledge of kidney disease for barriers affecting treatment adherence
  • Provide supportive counseling services to patients within scope of clinical training and state license
  • Provide information and assist with referrals to community resources (e.g., home health, vocational rehabilitation) for optimal treatment outcomes
  • Maintain knowledge of local vocational/educational rehabilitation programs and assist patients with referrals and access to remain/become employed or pursue education
  • Assess patient awareness of advance directives; assist with accessing forms/information and facilitate discussions with healthcare team, patient, family/support persons
  • Participate in discussions of patient DNR status to ensure informed decisions about care
