Requirements

  • Located in the state of Florida
  • Within reasonable commuting distance from posting location(s), unless accommodation granted as required by law
  • BA/BS degree required
  • Minimum of 2 years of experience working with a social work agency
  • Minimum of 5 years of experience working with individuals with chronic illnesses, co-morbidities and/or disabilities in a Service Coordinator or similar role (or any combination of education and experience providing equivalent background)

Responsibilities

  • Assists management with day-to-day activities, such as monitoring and prioritizing workflow
  • Performs reviews to ensure forms are completed accurately and timely
  • Assists in the development and maintenance of policies and procedures, training materials, and job resources
  • Leads projects and participates in workgroups with cross-functional teams; serves as a representative on business unit and enterprise initiatives
  • Performs process evaluations and tests; monitors systems/process enhancements
  • Manages overall member individual service plan within scope of position, as required by state law and contract; develops, monitors, and revises service plan to meet member needs and optimize health care across care continuum
  • Performs telephonic or face-to-face history and program needs assessments using predefined tools/questions for identification, evaluation, coordination, and management of long-term service and support needs (no clinical judgment involved)
  • Identifies members with potential clinical health care needs (e.g., high risk complications) using tools and predefined processes; coordinates cases with clinical healthcare management and interdisciplinary team for care coordination support
  • Manages non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost-effective and efficient utilization of long-term services and supports
  • Establishes short- and long-term service and support goals in collaboration with member, caregivers, family, natural supports, and physicians; identifies members benefiting from alternative service levels or waiver programs
  • May serve as mentor or preceptor for new staff, assisting in formal training
  • May be involved in process improvement initiatives
