Candidates must be a Licensed Nurse in good standing with the State Licensing Authority, have a minimum of 2 years’ experience in dementia care or managing two or more chronic diseases, and possess strong organizational skills, documentation skills, and the ability to communicate effectively.
The role involves fostering a positive work environment, serving as a mentor and leader, providing guidance and support to the community, and collaborating with physicians, therapists, families, and team members.
A strong candidate will have experience in training and team member development, the ability to establish effective relationships with residents, families, and team members, and skills in developing systems for monitoring resident health and ensuring safe care delivery.
