Responsibilities

  • Develop and process journey referrals while ensuring accurate documentation and workflow processes
  • Manage chart closure processes and provide documentation as requested
  • Work within various communication channels, including Teams, emails, HCHB, referral portals, SalesForce, and Medalogix
  • Assist other Journey team members with telephonic patient contact across multiple branches as requested
  • Demonstrate excellent customer service, both internally and externally
  • Understand and effectively communicate HIPAA compliance, corporate compliance, and client confidentiality
  • Communicate and collaborate with other Journey Team members, Palliative, Home Health, and Hospice teams as needed for internal team collaboration
  • Support/assist Hospice team members to ensure face-to-face visits are assigned appropriately and completed on time
  • Assist the Hospice Journey team with needs if they arise
