Requirements

  • Comfort utilizing psychological screening measures to assess treatment efficacy
  • Comfort with technology, including navigating electronic health records and registry platforms for tracking symptom improvement
  • Strong interpersonal skills and ability to work effectively in team-based environments
  • Ability to provide culturally responsive care

Responsibilities

  • Deliver evidence-based therapeutic interventions (e.g., CBT, PST, MI) to patients ages 12+ in primary care setting
  • Conduct comprehensive behavioral health assessments
  • Collaborate with patients and multidisciplinary care team (PCPs, nurses, psychiatrists) to develop and implement treatment plans
  • Communicate behavioral health principles to non-behavioral health professionals for collaborative treatment planning and care coordination
  • Provide patient-centered care, setting goals and developing plans reflecting patient preferences
  • Assess and manage behavioral health concerns and support coping with chronic health conditions
  • Respond to interruptions like warm hand-offs, urgent issues, and curbside consults
  • Utilize psychological screening measures to track symptom improvement and adjust treatment plans
  • Document efficiently in electronic health records in a clear, concise, timely manner
  • Expand access to quality care through innovative treatment approaches
