Able to cover primary locations: Nassau County surrounding area
Multi-state licenses (preferred)
Able to comfortably and safely perform duties in diverse home environments (e.g., household pets, secondhand smoke, airborne allergens or irritants)
Flexible to travel locally and within licensed states, as needed

Responsibilities

Conduct in-home health evaluations (approx. 45 minutes each), including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered (e.g., diabetic eye exams, spirometry, bone density screening)
Use iPad and intuitive clinical workflow for evaluations
Bring heart into every visit, building trusted relationships and improving health outcomes
Engage one-on-one, answer health questions, and provide education
Conduct virtual visits as needed
Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products
Fulfill requirements of the specified clinician role as part of the national network
