Able to cover primary locations: Chemung County, Elmira, NY
Able to comfortably and safely perform duties in diverse home environments (e.g., with household pets, secondhand smoke, airborne allergens or irritants)
Flexible to travel locally and within licensed states, as needed
Multi-state licenses (preferred)

Responsibilities

Conduct in-home health evaluations (approx. 45 minutes each), including medication and medical history review, physical evaluation, and answering health questions
Perform additional diagnostic and preventive services if ordered (e.g., diabetic eye exams, spirometry, bone density screening)
Use iPad and intuitive clinical workflow for evaluations
Conduct virtual visits as needed
Build trusted relationships and connect patients to appropriate care
Provide all Signify Health services as indicated
