Responsibilities

The Per Diem Physician will conduct in-home health evaluations, lasting approximately 45 minutes, including medication and medical history reviews, physical evaluations, and diagnostic testing as ordered by the person’s health plan. They will utilize an iPad and intuitive clinical workflow, engage in one-on-one conversations with patients, and provide connections to the right care, while also conducting virtual visits as needed and collaborating with Signify Health as a clinician colleague.
