Requirements

  • Must reside in the state of Ohio
  • Minimum of 3 years experience in developing housing and/or housing policy at a local, state, or federal level for special populations such as the elderly, people with disabilities, and/or people with mental health challenges to secure accessible, affordable housing through Federal and local programs (or equivalent combination of education and experience)
  • Within reasonable commuting distance from posting location(s) (e.g., Cleveland, OH), unless accommodation granted

Responsibilities

  • Develop and implement the health plan's housing strategy and programs, including assisting individuals in securing accessible, affordable housing through Federal and local programs
  • Possess knowledge of housing programs and services, focusing on homelessness, at-risk populations, and accessibility needs related to LTSS populations
  • Liaise between housing agencies and the health plan, partnering to develop and access affordable housing services for members
  • Partner with Health Plan trainers to develop and implement training specific to affordable housing services in Ohio
  • Work under the Housing First model, honoring member choice
  • Work with housing agencies and other housing programs to help develop and access affordable housing services for members receiving LTSS
  • Educate and support Care Coordinators and Support Coordinators in identifying housing options for members
