Requirements

  • High school diploma or GED equivalent preferred
  • Prefer six (6) months experience
  • Experience in caring for the terminally ill preferred
  • Reliable transportation that allows employee to shift work locations when needed
  • Furnish one or more active telephone numbers that enables management to contact employee should an unexpected change of work location be required
  • Current licensure as a Certified Nursing Assistant (CNA) in the state of Florida
  • Current State of Florida Driver’s license with driving record acceptable to Community Hospice & Palliative Care insurance carrier
  • Current CPR/BLS Certification
  • Immunizations or screenings as required

Responsibilities

  • Perform hands-on personal care
  • Perform simple procedures as an extension of therapy or nursing services
  • Provide assistance in ambulation or exercises
  • Report changes in the patient’s medical, nursing, rehabilitative, and social needs to a licensed nurse, as the changes relate to the plan of care and quality assessment and improvement activities
  • Complete appropriate documentation in compliance with the organization’s policies and procedures
  • Teach and train family/caregiver proper techniques to meet the personal care needs of the patient
  • Maintain patient confidentiality at all times
  • Attend team meetings and participate in other agency meetings as directed
  • Maintain a minimum of twelve (12) hours of training/education specific to position annually
  • Complete mandatory education per company policy
  • Other duties as assigned
