Requirements

Certified Medical Assistant (CMA) certification
Supervisory and team performance management experience
Experience in clinical team roles, particularly in intake and optimization processes
Ability to work in partnership with nursing and cross-functional teams
Strong organizational and communication skills
Ability to manage multiple inputs and systems, including medication lists, primary care, specialist, and pharmacy contacts
Experience with electronic records and medication audit management
Ability to work in a fast-paced environment and prioritize tasks effectively
Strong problem-solving and analytical skills
Ability to build rapport with care team members and drive operational initiatives

Responsibilities

Work in partnership with nursing and cross-functional teams to support daily operations and delivery of best care practices and standards
Supervise the work of individuals/teams and drive the success of operational initiatives
Manage member information, faxes, pharmacy calls, and systems documentation
Ensure execution of cross-collaborative work to capture, enter, and confirm health profile information vital for virtual on-boarding visits and/or support member on-boarding calls
Organize multiple inputs such as medication lists, primary care, specialist, and pharmacy contacts across different systems and software
Manage daily operations that support multiple providers, nurses, and health coaches to maintain care
Ensure accurate and reliable performance in managing member information and systems documentation
Collaborate with the nursing and cross-functional team leads to support the delivery of best care practices and standards
Support the optimization team in managing member information, faxes, pharmacy calls, and systems documentation
Drive the adoption of new processes, technologies, or in the care teams' coordination of care and successful achievement of clinical goals
