Requirements

  • Registered Nurse (RN)
  • Entry-level designation into the Clinical Advancement Program (CAP)
  • Ability to perform essential functions under the direction of a Supervisor
  • Ability to perform duties with guidance from Educator, Preceptor, or in collaboration with another RN
  • Ability to collect and analyze dialysis data and document in the patient medical record
  • Ability to assess and collaborate on patient/family learning needs
  • Ability to administer medications as prescribed or in accordance with approved algorithms
  • Ability to initiate or assist with emergency response measures
  • Ability to serve as a resource for the healthcare team and participate in staff training
  • Ability to ensure correct laboratory collection, processing, and shipping procedures
  • Ability to identify expected outcomes and update the nursing assessment and plan of care
  • Ability to assess barriers in the home environment and partner/family readiness
  • Ability to train patients and/or Home Partners on the operation and maintenance of Home Dialysis equipment
  • Completion of CAP requirements to advance

Responsibilities

  • Provide and coordinate clinically competent care for assigned patients, including assessment, planning, intervention, and evaluation
  • Assess and manage patients’ response to home dialysis training and treatment therapy
  • Communicate patient-related issues to the physician as needed
  • Participate in decision-making, teaching, leadership functions, and quality improvement activities
  • Perform ongoing collection and analysis of dialysis data
  • Provide initial and ongoing education to patients and families
  • Direct and provide safe and effective delivery of dialysis therapy
  • Assist with emergency response measures
  • Serve as a resource for the healthcare team and participate in staff training
  • Ensure correct laboratory collection, processing, and shipping procedures
  • Identify expected outcomes and update the nursing assessment and plan of care
  • Ensure patient awareness related to transplant and treatment modality options
  • Assist in the identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
  • Train Home Dialysis patients and/or Home Partners on the operation and maintenance of Home Dialysis equipment
  • Perform all other duties as assigned by Supervisor
