Registered Nurse (RN)
Entry-level designation into the Clinical Advancement Program (CAP)
Ability to perform essential functions under the direction of a Supervisor
Ability to perform duties with guidance from Educator, Preceptor, or in collaboration with another RN
Ability to collect and analyze dialysis data and document in the patient medical record
Ability to assess and collaborate on patient/family learning needs
Ability to administer medications as prescribed or in accordance with approved algorithms
Ability to initiate or assist with emergency response measures
Ability to serve as a resource for the healthcare team and participate in staff training
Ability to ensure correct laboratory collection, processing, and shipping procedures
Ability to identify expected outcomes and update the nursing assessment and plan of care
Ability to assess barriers in the home environment and partner/family readiness
Ability to train patients and/or Home Partners on the operation and maintenance of Home Dialysis equipment
Completion of CAP requirements to advance

Responsibilities

Provide and coordinate clinically competent care for assigned patients, including assessment, planning, intervention, and evaluation
Assess and manage patients’ response to home dialysis training and treatment therapy
Communicate patient-related issues to the physician as needed
Participate in decision-making, teaching, leadership functions, and quality improvement activities
Perform ongoing collection and analysis of dialysis data
Provide initial and ongoing education to patients and families
Direct and provide safe and effective delivery of dialysis therapy
Assist with emergency response measures
Serve as a resource for the healthcare team and participate in staff training
Ensure correct laboratory collection, processing, and shipping procedures
Identify expected outcomes and update the nursing assessment and plan of care
Ensure patient awareness related to transplant and treatment modality options
Assist in the identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
Train Home Dialysis patients and/or Home Partners on the operation and maintenance of Home Dialysis equipment
Perform all other duties as assigned by Supervisor
