Responsibilities

Manage end to end patient care to ensure patients achieve their health goals
Consult new patients on the platform to educate them about the Twin program, health conditions, health goals & review medical reports & prescriptions
Supervise & encourage patients to achieve their health goals (example reversal of diabetes)
Encourage patients to follow the program guidelines & adhere to their health & nutritional requirements
Retain patients on the program by hand holding them & encouraging to move towards their health goals
Own patient outcome for your assigned set of patients
Communicate effectively with partner doctors and patients
Manage all patients under the guidance of partner doctors & senior twin doctors
Manage patients in the care of senior doctors & partner doctors to deliver health outcomes
Manage all patient escalations round the clock to ensure highest level of patient safety
Follow all Twin protocols & guidelines
Gain solid knowledge of company product & metabolic disease reversal
Collaborate with internal team (coaches, operations, doctors) to achieve results
Be available in all shifts (morning/evening/night) as per requirement
