Requirements

  • Board certified in Internal Medicine and Board Certified or Board Eligible in endocrinology (new fellowship graduates welcome)
  • Strong belief in an innovative, value-based approach to diabetes care focused on clinical quality, patient experience, shared decision making, and improved access to endocrinology services
  • Comfortable delivering virtual-first diabetes care through telehealth appointments, remote monitoring, in-home diagnostics, and collaboration with community primary care providers (PCPs)
  • Agile mindset open to a high impact, novel care model which may undergo rapid iterations to achieve continued improvements in clinical and quality outcomes
  • Prior experience practicing in value-based care models
  • Willingness to obtain licensure in all Devoted Health markets (concierge support provided)
  • Enjoy a fast-paced, high-energy organization where agility and collaboration are key

Responsibilities

  • Provide best-in-class direct telehealth care in the virtual Diabetes Clinic (approximately 80% of time)
  • Lead clinical management of complex patients with diabetes and its associated comorbidities
  • Provide clinical supervision and project-based work to help build out the Diabetes Clinic care model (approximately 20% of time)
  • Utilize and help improve home-grown technology and electronic health information platform for visits (initial visits 45-60 minutes)
  • Partner with care coordination team to help members access high-quality in-person endocrinology care when required or preferred
  • Serve as backup to nurse practitioners on call for the care on demand clinic for approximately one weekend per quarter (no overnight call responsibilities)
