Responsibilities

  • Develop and lead condition-based care programs aligned with organizational goals, including:
  • Virtual Telehealth: Leading a virtual care team that supports patients with chronic conditions through telephonic and video-based visits
  • Heart Failure Management: Driving heart failure screening, management and intervention strategies to reduce admissions
  • Chronic Disease Programs: Enhancing in-center workflows to improve diabetes and hypertension screening and ongoing management
  • Vaccines: Developing and scaling comprehensive strategy for vaccine administration across Oak Street Health
  • Other: Support the creation and expansion of additional condition-based programs as organizational needs evolve
  • Identify and implement strategies to improve medical cost efficiency, quality performance, and Stars measures
  • Lead cross-functional collaboration with clinical operations/population health, medical leadership, and product teams to ensure successful operational execution
  • Monitor and analyze program performance, leveraging data insights to drive continuous improvement and alignment with KPIs
