Requirements

Bachelor’s degree in a human services field
Successful completion of background check including criminal record, driving record, abuse/neglect, and fingerprint check
Completion of New Hire Orientation at the beginning of employment
All training requirements including Relias at the beginning of employment and annually thereafter
Current driver’s license, acceptable driving record, and current auto insurance
Ability to perform sedentary work: exerting up to 10 pounds of force occasionally, negligible force frequently; repetitive movements of hands, fingers, and arms; sitting most of the time with occasional walking or standing

Responsibilities

Respond to inquiries for mental health, domestic violence, and/or addictive disorder services and refer to appropriate system programs, outside agencies, or community resources
Help locate resources, apply for services, set appointments, provide accurate information, advocate, and follow up when necessary
Maintain a system of records and reports for documentation of referrals at specific time intervals
Provide interim triage contact with consumers and assist with connecting to behavioral and crisis intervention services
Deliver supportive services to assigned consumers until they establish care with providers for ongoing treatment needs
Provide and/or coordinate follow-up to all responsible consumers, including for missed appointments, and conduct outreach and engagement as needed
Collaborate as part of a team which may include hospitals, medical clinic health professionals, or other employees of a designated community partner
Attend individual supervision and team meetings as scheduled by the ERE Supervisor
Follow all policies and procedures as outlined by the system, the Missouri Behavioral Health Council, and the Department of Mental Health
