Requirements

  • Part-time availability (20 hours/week in Cayey)

  • Ability to assess patient nutritional status, adequacy measures, fluid status, and nutrition-related lab parameters
  • Skills in calculating diet prescriptions according to standards of practice
  • Proficiency in completing nutritional assessments and progress notes per FMCNA guidelines and federal/state regulations
  • Capability to monitor patient adherence, albumin levels, body weight, and adjust nutrition therapy
  • Experience collaborating with interdisciplinary teams on Urea Kinetic Modeling, mineral metabolism, renal bone disease, anemia, iron status, psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Ability to identify malnourished patients and recommend interventions
  • Skills in patient/family/caregiver education on nutritional status and Pre-ESRD education
  • Adherence to FMCNA algorithms, medical record policies, and QAI activities

Responsibilities

  • Assess patient’s knowledge of diet and kidney disease and provide tailored education
  • Deliver patient-specific education on nutritional status, adequacy measures, fluid status, and lab parameters
  • Complete nutritional assessments and participate in interdisciplinary comprehensive assessments
  • Develop written, individualized comprehensive plans of care per federal/state guidelines
  • Calculate diet prescriptions and complete progress notes on chronic dialysis patients
  • Maintain accurate documentation in medical records per FMCNA policies and regulations
  • Provide ongoing nutrition assessment and counseling to achieve effective nutritional status
  • Identify and address malnutrition risks collaboratively with the interdisciplinary team
  • Review monthly albumin levels and body weight; implement plans of care with patient/family involvement
  • Monitor therapy adherence, adjust as needed, and document barriers to goals
  • Collaborate on Urea Kinetic Modeling, mineral metabolism, anemia, nutritional/psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Report on nutrition QAI results and participate in interdisciplinary QAI programs
  • Coordinate Pre-ESRD education and refer for nutritional supplements as appropriate
  • Review nutrition-related lab results, counsel patients/caregivers, recommend treatment changes, and communicate with physicians/staff
  • Participate in pertinent staff meetings
