Responsibilities

  • Direct Patient Care Technicians in providing safe and effective chronic hemodialysis therapy in compliance with facility policies, procedures, and regulations
  • Delegate tasks to direct patient care staff including LVN/LPNs, Patient Care Technicians, and Dialysis Assistants
  • Assess daily patient care needs and develop appropriate patient care assignments
  • Routinely monitor patient care staff for appropriate techniques and adherence to facility policies and procedures
  • Participate in staff training and orientation of new staff as assigned
  • Participate in all required staff meetings as scheduled
  • Ensure educational needs of patients and family are met regarding End Stage Renal Disease (ESRD)
  • Provide ongoing education to patients regarding renal disease, vascular access, dialysis therapy, diet/fluid and medication compliance, adequacy measures (OLC, AMP, UKM), transplant/modality awareness, catheter reduction, laboratory values, and treatment compliance
  • Provide safe and effective delivery of care to patients with ESRD
  • Accurately implement treatment prescriptions including Sodium (Na) modeling and Ultrafiltration modeling
  • Assess patients’ response to hemodialysis treatment, make adjustments/modifications as indicated, and communicate issues to Team Leader or physician
  • Identify and communicate patient-related issues to Team Leader or physician
  • Initiate Initial and Annual Nursing Assessment, and ongoing evaluation and documentation of patient care needs
  • Actively participate in pre-evaluation, initiation, monitoring, termination, access homeostasis, and post-evaluation of hemodialysis treatments
  • Take appropriate interventions for changes in patient adequacy status and troubleshoot access flow issues
  • Provide, supervise, and monitor hemodialysis access care
  • Implement, administer, monitor, and document responses to prescribed interdialytic transfusions, notifying physician and blood supplier of adverse reactions
  • Ensure accurate and complete documentation by Patient Care Technicians on the Hemo system
