Requirements

Candidates must possess a minimum of 1 year of experience as a Nurse Practitioner within the D2Me program. A valid Nurse Practitioner (APRN) license in good standing in the required states, or the ability to obtain multi-state licensure within 90 days of hire, is essential. Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine is required, along with a minimum of 2 years of experience in primary care, urgent care, or internal medicine, preferably with telehealth experience. Active BLS certification at the time of hire is also mandatory. Flexibility with shift times, including potential future weekend shifts, is necessary.

Responsibilities

The Virtual Primary Care Nurse Practitioner will provide comprehensive primary care services via telehealth, including patient assessments, chronic disease management, preventive screenings, and acute care interventions. Responsibilities include diagnosing and managing chronic conditions, evaluating and treating acute concerns, ordering and interpreting diagnostic tests, and managing patient medications. The role involves providing health coaching, patient education, and facilitating referrals or home-based services as needed. Additionally, the Nurse Practitioner will utilize and provide feedback on the electronic health platform, participate in clinical case reviews and interdisciplinary collaborations, and engage in continuous learning to stay updated on best practices in virtual care.
