Requirements

  • Must be in the Connecticut or Massachusetts area (onsite position)
  • Expertise in Continuous Quality Improvement (CQI) principles, methods, and tools
  • Effective communication skills among interdisciplinary teams (IDT)
  • Knowledge of all treatment modalities (in-center, home hemodialysis, home peritoneal dialysis)
  • Ability to monitor data/information, prioritize improvement areas, determine root causes, and develop/implement/evaluate/revise improvement plans
  • Subject matter expertise in clinical quality matters
  • Knowledge of CMS Conditions for Coverage, FMS/FKC quality programs, Value Based Payer Programs, and quality improvement concepts/principles/practices
  • Proficiency in data collection, trending, and analysis
  • Familiarity with evidenced-based practices, Corporate Medical Advisory Board Recommended Algorithms/Standing Orders, clinical pathways, policies, and procedures
  • Ability to leverage tools, resources, and informatics technology

Responsibilities

  • Develops, implements, maintains, and evaluates data-driven quality assessment and performance improvement programs with interdisciplinary team participation
  • Serves as quality improvement champion and role model, promoting CQI to improve processes and patient outcomes at facility and area levels
  • Facilitates implementation and integration of FKC Quality programs/initiatives into facilities' standard operating procedures via teamwork with clinical, interdisciplinary, and operations teams
  • Identifies clinical quality improvement opportunities to achieve CMS Conditions for Coverage and FMS quality requirements
  • Manages tactical execution of Quality and clinical initiatives, interventions, and standardized education with facility RNs
  • Provides nursing/clinical guidance, theoretical perspectives, and feedback to facility RNs on quality, patient satisfaction, teamwork, unit culture, and employee satisfaction
  • Collaborates with stakeholders (Clinical Quality leadership, Education, Clinical Services, Regulatory, Compliance) to achieve quality goals and patient safety improvements
  • Mentors/trains staff to collect, trend, and analyze data daily to monitor processes, improve patient outcomes/satisfaction, and reduce morbidity/mortality
  • Utilizes evidenced-based/best practices to address quality barriers and promote adoption of algorithms, standing orders, pathways, and procedures
  • Leverages tools/resources/informatics to target patient populations
  • Applies knowledge of FMS policies, CMS Conditions, and QI practices to perform duties
  • Promotes collaboration between facilities/areas to share processes/strategies, mentors on effective practices, and assists in testing/adoption/implementation
