Requirements

Candidates must be independently licensed mental health professionals, such as LCSW, LMFT, LPC, or LICSW. Flexibility is required for occasional shift adjustments, and all After Hours team members are required to work on holidays that fall on their scheduled work days. The position requires availability to work specific shifts including Tuesday through Saturday from 9 PM to 5 AM PST, 11 PM to 7 AM CST, or 12 AM to 8 AM EST.

Responsibilities

The Clinical Care Navigator will provide in-the-moment clinical support, psychoeducation, and connection to appropriate referrals and resources while answering calls on a clinical phone queue. They will utilize skills such as active listening, motivational interviewing, and solution-focused approaches to collaboratively engage with members seeking help with mental health, substance use, or family/relationship concerns. The role involves assessing for risks, providing crisis support, safety planning, and follow-up with high-risk members. Recommendations for a plan of action will be made based on clinical presentation, and coordination of connection to appropriate care and services will be performed, including Spring Health therapists, coaches, physicians, or external resources. Effective written skills will be used for professional documentation and member/provider interactions through email, and adherence to care management outreach protocols and utilizing good clinical judgment to identify and problem-solve barriers or disruptions to care is expected.
