Requirements

  • North Carolina residency required
  • Licensed clinical professional (within the scope of licensure for case management in special programs like Foster Care)
  • Ability to travel within assigned Region 1 counties (Buncombe, Cherokee, Clay, Graham, Haywood, Henderson, Madison, Polk, Swain, Transylvania, Yancey, Macon, Jackson, Mitchell)
  • Field-based role requiring travel to client sites or designated locations, with occasional office attendance; work virtually from home when not in the field
  • Within reasonable commuting distance from posting location(s), unless accommodation granted
  • Availability for standard business hours, Monday through Friday

Responsibilities

  • Perform case management telephonically and/or by home visits within scope of licensure for special programs like Foster Care
  • Manage overall healthcare costs for designated population via integrated (physical health/behavioral health) case management and whole person health
  • Support behavioral health requirements of children and adolescents in North Carolina's foster care system by coordinating care, facilitating mental health service access, and collaborating with providers, case workers, and families
  • Educate families and case managers about behavioral health conditions and treatment options
  • Conduct assessments to identify individual needs
  • Develop comprehensive care plans to address objectives and goals identified during assessment
  • Support member access to appropriate quality and cost-effective care and modify plans as needed
  • Coordinate with internal and external resources to meet identified needs in terms of integrated (physical and behavioral) whole person care
  • Coordinate social determinants of health and incorporate into care planning
  • Work closely with various state agencies
  • Maintain knowledge of the system of care philosophy and coordinated network of community-based services
  • Build meaningful partnerships with designated populations and families, addressing cultural and linguistic needs
  • Evaluate health needs and identify applicable services and resources in conjunction with members and families
  • Provide patient education, medication reconciliation, identification of community resources, and assist with arrangement of follow-up
